IV. Neuroscientific Substrates of the Mismatch Signature

4.1 The Social Pain Network and Its Dysregulation

The neurobiological basis of social pain — the experience of rejection, exclusion, and relational loss — has been substantially illuminated over the past two decades. Eisenberger and Lieberman’s foundational neuroimaging work demonstrated that social exclusion activates the dorsal anterior cingulate cortex (dACC) and anterior insula — regions also implicated in the experience of physical pain (Eisenberger et al., 2003). This overlap supports the “social pain overlap theory,” which holds that social rejection is processed through neural architecture originally evolved for physical threat detection.

In individuals with ADHD — and by extension, those with RSD — several neurobiological features are relevant to understanding the Mismatch Signature. First, the dACC, which plays a critical role in conflict monitoring and the detection of discrepancies between expected and actual outcomes, shows atypical activation patterns in ADHD (Bush et al., 2005). This is directly relevant to the Mismatch Signature: if conflict monitoring is impaired, the threshold at which a neutral stimulus is flagged as a rejection-relevant discrepancy may be substantially lowered.

Second, dopaminergic dysregulation — well-established in ADHD (Volkow et al., 2009) — disrupts the reward prediction error signal that normally calibrates emotional responses relative to expected outcomes. In neurotypical processing, a mismatch between expected social warmth and received neutral or ambiguous social input generates a mild prediction error, prompting modest affective recalibration. In the context of dopaminergic dysregulation, this same mismatch may generate an amplified, poorly calibrated prediction error — producing the disproportionate internal response characteristic of the Mismatch Signature.

Third, the amygdala’s role in threat detection and emotional amplification is relevant. Individuals with ADHD show evidence of amygdala hyperreactivity to emotional stimuli (Posner et al., 2011), particularly in contexts involving social evaluation. The Mismatch Signature may be partly understood as the behavioral and phenomenological expression of an amygdala that treats ambiguous social cues with the same urgency as genuine threat signals — triggering the full cascade of stress response, emotional flooding, and behavioral disruption without the social calibration that would normally occur.

4.2 The Role of Norepinephrine: Emotional Volatility and Signal Amplification

The noradrenergic system — particularly dysregulated in ADHD and implicated in the efficacy of norepinephrine-targeting medications like atomoxetine and guanfacine in RSD treatment (Dodson, 2016) — provides another neurobiological window onto the Mismatch Signature. Norepinephrine plays a key role in signal-to-noise regulation in neural processing: it amplifies relevant signals while suppressing irrelevant background noise. In contexts of noradrenergic dysregulation, this function may be impaired — with minor social signals being amplified into foreground events of compelling urgency, while the regulatory background context that would otherwise situate them as minor is suppressed.

This neurochemical dynamic maps remarkably well onto the Mismatch Signature. The person experiencing an RSD episode is not misperceiving the social cue in a purely cognitive sense — they may accurately recognize that the cue is minor or ambiguous. What the Mismatch Signature captures is that despite this recognition, the affective and neurobiological response proceeds as if the cue were major and definitive. The noradrenergic signal amplification system has, in effect, promoted a minor cue to major status in the processing hierarchy, generating a response calibrated to the amplified rather than the actual signal.

What shows up

A response of great magnitude to a cue of small magnitude — fast in onset, bewildering to the person having it.

What it protects

Belonging. The part that reads faces early is standing in front of every exclusion that has already happened once.

The gift inside it

Extraordinary social resolution — a capacity for attunement, running at a gain it was never given a way to lower.
